Privacy Collection Notice
The Department of the Premier and Cabinet (DPC) collects information submitted through this form to:
- understand user experiences with the ServiceWA app;
- identify opportunities to improve the ServiceWA app and related services;
- respond to your feedback where you have requested a response; and
- maintain records of feedback received about the ServiceWA app.
DPC may disclose your information to relevant DPC staff, contractors and service providers who assist with the operation, support and improvement of the ServiceWA app.
Providing feedback is voluntary. If you request a response but do not provide your contact details, DPC may be unable to respond to your feedback.
Please avoid including personal information about yourself or other individuals in your comments.
