PRELIMINARY EVENT NOTIFICATION
Purpose of Document
This Preliminary Event Notification (PEN) form is used to gather data and information to determine if a disaster event meets the activation criteria of the Disaster Recovery Funding Arrangements Western Australia (DRFAWA).
Requirements to activate the DRFAWA
To be eligible for assistance under the DRFAWA, a disaster event must meet all three of the following criteria:
- The event was a natural disaster or a terrorist act.
A natural disaster includes one or more of the following rapid-onset events: bushfire, earthquake, flood, storm, cyclone, storm surge, landslide, tsunami, meteorite strike, and/or tornado.
- A coordinated, multi-agency response was required.
- Eligible expenditure exceeded $240,000 across the affected local governments.
This includes costs related to eligible emergency assistance, damage to Essential Public Assets (EPA), and/or other eligible assistance provided in response to the event.
Supporting evidence requirements for the PEN Form
The information provided in the PEN form must be supported by verifiable evidence and uploaded in the Supporting Evidence section. Supporting evidence may include:
- Weather data such as rainfall, wind, or other relevant metrics from the Bureau of Meteorology (BoM) or the Department of Primary Industries and Regional Development (DPIRD).
- Trusted and verifiable media reports that confirm the event and its impacts.
- Emergency WA warnings and incident alerts related to the disaster event.
- Records of coordination and communication, including minutes of meetings, phone call logs, emails, or other documentation that demonstrate a coordinated multi-agency response or confirm impacts.
- Visual evidence, such as dated, geotagged, and clearly labelled photographs (with metadata) showing the impacts or damage.
- Public notifications regarding impacts on Essential Public Assets (e.g., roads, bridges) or response activities. These may include posts on Local Government (LG) or State Government Agency (SGA) websites or social media channels.
Activation of an event does not constitute approval of funding. If an event is activated, all subsequent Essential Public Asset applications or claims must include sufficient, relevant and reliable photographic evidence. This evidence must:
- be dated and geotagged
- be clearly labelled
- include complete embedded metadata (e.g. location, date and time)
- clearly demonstrate both the pre-disaster and post-disaster conditions of the asset
- be sufficient in number to clearly demonstrate the extent and scope of both the pre-disaster condition and the damage. As a guide for roads, the recommended maximum linear distance between photographs along continuous sections of damage should be 50 meters.
Evidence of expenditure and, where applicable, procurement will also be required.
If you do not have the evidence described above, contact DFES Recovery Funding to discuss the requirements before submitting a PEN form.
Other forms of evidence may be required for assistance types unrelated to Essential Public Assets. Contact DFES Recovery Funding to discuss the requirements relevant to your circumstances, or refer to the factsheets and other resources on the DFES Recovery Funding webpage for information about assistance measures and evidence requirements.
