    This form must be completed and submitted to DFES at least four (4) weeks prior to the proposed test date.

     

    • A PDF copy of the proposed block plan must be included with this submission.
    • DFES will exercise reasonable care when discharging water during hydrant system testing. However, DFES accepts no responsibility or liability for any damage (actual or perceived) arising from the testing process. Responsibility for any such damage rests with the person in control of the site.

     

    ⚠️ Important Notice

    All Fire Hydrant Booster System Hydrostatic Test requests are subject to review and approval on a case-by-case basis. Submission of this form does not guarantee that testing will be approved or scheduled.
